What's the point?
We are doing this questionnaire because we want some information that will help our school make sure we give you a programme of health education that is right for you and other people in the school. The information will also help the NHS in planning health care and health education for young people.
Who is behind it?
The survey is being organised by the local NHS, Children's Services and other organisations concerned with young people.

Prices can can from less than £100 to carry out a primary school parent perception survey online, to the best part of £1000 to do a full lifestyle survey with secondary school pupils in two year groups.

We can set up an online survey for you now, or send you paper questionnaires in the post tomorrow.
How long does a survey take to complete?
The shortest questionnaires we offer can be completed within 15 minutes, while our longer lifestyle questionnaires can take up to an hour. Online, the computer will skip pages that are not relevant.
